About

Community Hospice & Palliative Care provides services to eligible patients in the last year of their life. We take care of patients in their homes to improve the quality of life at the end of life. All services are FREE to patients and families and include medical equipment needed for their care such as a hospital bed, bedside table, bedside commode, shower chair, oxygen, medications related to their diagnosis, and supplies such as adult diapers and wound care items.
